I had trouble as well. The first time I downloaded the form it was fine, but then I rebuilt my computer and downloaded it again a few weeks later. Now it’s funky. I don’t know if it was edited or if I’m just lucky. Anyway, I changed a bit of the formatting on the page to fix it. I’d be happy to send you a copy of the one I repaired. Hopefully it opens ok on Dr. Diana’s end.
