I snail mailed my symptoms checklist last week to the address on the contact page (copy and pasted below)and it was returned in my snail mail today as “undeliverable as addressed and unable to forward”. I have it addressed exactly as it is below. What is up? HELP! I’ve posted on other boards to snail mail it to this address too. Yikes!
